PREPARING FOR YOUR APPOINTMENT

Preparing for your appointment can save you time and help us to better help your pet!

1) Please call your veterinarian's office and request that they fax us your pet's medical record
2) Please prepare a list of any medications your pet has used or is currently using (including ear drops, sprays, shampoos, pills, dietary supplements)
3) Please fill out the dermatology history form - faxing it back prior to the day of the appointment will also save your time!
